The Mysterious TMJ(D)

Mention "TMJ" to people, and almost to a person everyone either knows someone who has this disorder, or they have it themselves. Despite the millions of people who suffer from TMJD (Temporo Mandibular Jaw Disorder) there is mostly just educated speculation as to the possible causes of this disorder. Even less is certain about its cure.

The temporo mandibular joints are small ball-and-socket hinge like structures on each side of your head that essentially connect your lower jaw to your skull. The lower jaw has rounded ends (condyles) that smoothly move in and out of this socket when one eats, speaks, yawns, etc. The surfaces of these bones are covered with cartilage, so bone is not grinding on bone, and the surfaces are also separated by a small disk which acts as a shock absorber. Sixty-eight pairs of muscles in your face and jaw area work to help stabilize this joint so that it moves smoothly to ensure speaking, swallowing, chewing, breathing, and even keeping your head on your shoulders! For the person with TMJD, however, any or all of these movements can become extremely painful as the condyles become dislocated or the cartilage excessively worn or overly stretched.

Researches now divide the disorders of the temporomandibular joint into three categories: one is myofascial pain, and it is the most common. This pain is characterized by discomfort or pain in the muscles that control jaw function and the neck and shoulder muscles. Then there is of the internal derangement of the joint, which means the jaw is dislocated, the disc is displaced, or the condyle is injured. The last category is degenerative joint disease, such as arthritis in the jaw joint. (See reference below.)

This much science knows, but the certainty seems to stop here. The causes of TMJD seem to be as variable as the people who have it. Actual numbers of people experiencing TMJ disorders fluctuate widely, ranging from five to fifteen percent of the population of the United States to more than thirty-five percent of people. Women are more likely to develop this condition than men…and no, excessive talking is not to blame, although talking can cause pain if one has this disorder!

Amazingly, many suffering from a TMJ ailment seem to know the exact cause of his/her disorder. Some develop this condition due to facial or head injuries because of blows to the head, jaw or face, or from whiplash A great many sufferers indicate that they developed TMJ pain after prolonged dental procedures. Could it be your bite? Maybe your spine is misaligned. Do you need orthotics in your shoes? Perhaps there is a genetic connection. NIDCR is conducting a study to identify biological or genetic factors that may contribute to this disorder. Another recent study suggests there is a link to serotonin levels. While it would seem very reasonable to blame grinding and clenching of teeth for TMJD, ironically many people who grind and clench never experience TMJ pain. Arthritis can certainly be a cause, as can simply having tight jaw muscles due to stress.

Overuse of the jaw is another possible cause. Excessive gum chewing, nail biting and gnawing on items like plastic straws can overtax the muscles of this joint.

You may be experiencing TMJ problems if your jaw clicks, pops, catches, or locks when you open your mouth, Frequent headaches, neck-aches and ear pain and ear infections are other indicators of TMJ joint disorder. When TMJ affects the ear, one may then experience dizziness, lightheadedness, ringing in the ears, and a fullness or pressure on the ear drum. (Perhaps you don't need those antihistamines after all.)

Pain from the jaw can also radiate down your shoulders into your arms. Your teeth may not line up when you close your mouth, and you may not be able to open your mouth fully. In fact, the old song about the leg bone being connected to the hip bone, being connected to the back bone is an entirely appropriate analogy of TMJ pain. You may experience aches and pains throughout your body as your jaw struggles to align itself.

Fortunately, most people suffering from TMJ disorder will recover without medical intervention. Others will recover but will continue to have periodic bouts of the condition. Sadly, there are those for whom the pain of this disorder will become an indelible black mark on the fabric of their lives. Those with osteoarthritis or rheumatoid arthritis in the jaw joint may not experience total recovery either. Whether your case of TMJD is fleeting or permanent, experts agree that one of the most important things you can do is to ease the stress on the jaw joint, and this can be done through dietary changes. This does not mean one must subsist on oatmeal, soup, or pureed foods for six months to a year, and that is how long (or longer) it may take for your jaw to fully recover. It does mean, however, that a diet that is easy to chew will be most helpful for your condition.

Other possible courses of action for improvement involve physical therapy, chiropractic work, applying heat or ice packs, massage therapy, hypnotism, and avoiding extreme jaw movements (such as wide yawning, singing, gum chewing, screaming, etc.) Learning stress reduction techniques is another possible course of action, as well as short-term use of pain relievers. Mouth guards are widely prescribed by many dentists, but one must remember that mouth guards are not a cure for TMJ disorder. Mouth guards will help prevent damage to one's teeth if one is grinding and clenching excessively, especially at night. Mouth guards, however, do not address the fundamental issue of WHY one is clenching and grinding. For some people, wearing a foreign object in one's mouth can cause an increase in clenching and grinding and resulting pain.

Be cautious with this disorder. More medical treatment is not necessarily better. Some dental experts urge that TMJD treatment be conservative and reversible. "Conservative treatments do not invade the tissues of the face, jaw or joint. Reversible treatments do not cause permanent, or irreversible, changes in the structure or position of the jaw or teeth." (ibid). Always seek a second opinion if you are advised to have orthodontics to change the bite, or crowns, bridge work or grinding the teeth down to balance the bite. "Scientists have learned that certain irreversible treatments, such as surgical replacement of jaw joints with artificial implants, may cause severe pain and permanent jaw damage. Some of these devices may fail to function properly or may break apart in the jaw over time." (ibid).

TMJ disorder can be an extremely painful condition that can affect every moment of your life, not just when you're eating. Sleep can be disturbed, work can be compromised, activities are affected (whether they be sports, exercise, singing, or playing wind instruments) participation in group outings can be affected, and your dental health as well as your overall health and well being can suffer.

Remember the old expression, "patience is a virtue, possess it if you can..." Most of us are sadly lacking in this virtue and want a quick fix, but patience, time, a softer diet, and stress management will go a long way in easing the pain of this disorder for the majority of people who suffer from it.

Diabetes Epidemic Threatens Health Care System

Cardiovascular disease, strokes, and peripheral vascular disease are all very expensive conditions to treat in our technologically sophisticated health care system. These conditions already occupy some of the top slots when it comes to where our health care dollar are spent. So imagine what is going to happen now that the rates of diabetes have doubled (with no end in sight).

So, this epidemic of diabetes and pre-diabetes is not just a health care issue that burdens individuals and their families living with the disease. It is a looming societal problem that threatens to bankrupt our already fragile health care system. It could cause health care insurance premiums to escalate even further, impacting not only employers who provide coverage, but also public payors, like Medicaid and Medicare.

An article published in the New York Times January 31, 2007 by Richard Pérez-Peña declares, "One in Eight Adults in the City Has Diabetes, a Study Finds". That is 12.5% of the population or 700,000 people. Lest you feel relieved that you don't live there, let me remind you that the rest of the country is not all that far behind. Overall, about 10.3% of Americans have diabetes and about a quarter of them don't know it (yet).

An additional 24% of adults in NY (and in the rest of the country) have abnormally high blood sugars that have not yet reached diabetic levels. This condition is known as pre-diabetes.

Not too many years ago, the diabetes rate was 6%. But our self-indulgent ways have caught up with us. Too little exercise, too many calories plus too much stress adds up to an epidemic of obesity, particularly abdominal obesity. Abdominal obesity, especially visceral obesity, is linked to the development of Type 2 diabetes in genetically predisposed individuals.

It's just a "touch of sugar." Why all the concern? Because diabetes and it precursor, pre-diabetes, are the most familiar manifestations of a constellation of metabolic changes, known as cardiometabolic syndrome. Other manifestations of this syndrome are high blood pressure, dyslipidemia ((high triglycerides) and low HDL ("good") cholesterol)), clotting abnormalities, and problems with inflammation. People with cardiometabolic syndrome, even if they haven't yet developed full-blown diabetes, have an increased risk of heart attacks and strokes.

It is time to get deadly serious about doing something about prevention. We need to rapidly move to institutionalize regular exercise programs in school and at work. And we need affordable, easily accessible healthy eating options. That means we must be willing to regulate, legislate, and maybe even implement taxes (oh, oh, the tax word!) that can get us to where we need to be. Failure to do something now will almost certainly result in huge adverse financial consequences that will ripple through society in the not too distant future.

It will not do to get cavities on top of having to wear the braces. The brackets and braces that are used will cover the exterior of the teeth but the wires attached to them will be something that will definitely get in the way.

You may have to give your child an incentive of some kind to get them motivated to accomplish this task. How To Floss Properly

Flossing is one of the most important things you can do for the health of your teeth. So, how do you floss properly?

Start by using the correct amount of floss. You will want close to 18 inches so that you have enough to wind around your fingers, and still have about two inches to use to clean in between your teeth.

Next, is how you hold the floss. You must wrap your floss around your middle finger on each hand. You must then hold it taut so that the floss inserted between your teeth sits tight, and cleans thoroughly.

After you have the hold right, you will want to make sure that you insert the floss between your teeth correctly. You do not want to snap the floss in between your teeth. If you snap it in, then it can cause damage to the gums. It can make your gums bleed, and can be fairly painful. So, instead, gently wiggle the floss into the spaces between teeth.

Once you have the floss in between your teeth, you need to use up and down motions, not side to side, to remove plaque and food particles. You will want to go down the gumline, under the gum about a millimeter or two, and then lift up. By doing this you remove the plaque and debris that cause tartar and tooth decay under the gum.

You will want to do this between each tooth, and you will want to be sure to do a good job on rear teeth so that in hard to reach places, you still get a clean floss. This is important or you will get cavities.

After you floss your teeth you want to be sure to brush. Most people floss after brushing, but the opposite is the best way to floss properly. Why? Flossing helps to loosen plaque and debris, but not all of it is swept clean away by the floss, so if you brush, or swish with mouthwash after you get a more effective clean.

When combined with brushing, flossing is the most effective way of keeping your teeth free of decay and plaque build-up. So, no matter how much you brush, be sure to floss as well.

Experts recommend you floss at least once a day.

Before you begin flossing, find a type of floss you will like to use, and one that is the right size and kind for your teeth. If you have large spaces between your teeth, a woven floss or dental tape may work best. If your teeth are crowded, or close together, a waxed floss that will slide between them easily is best. There are many varieties of floss available, including waxed, unwaxed, and flavored, weaved, etc. Finding the floss you enjoy the most will make your flossing experience better. You will be more likely to floss daily, and possibly get an even better clean.

The majority of people prefer waxed floss, as it slides in and out easier and is less likely to shred as you floss your teeth. However, it is totally a personal preference, so try a few varieties out and see which one works best for you. Whatever type you decide use is your personal preference, but make sure the floss is approved by the ADA (American Dental Association.)

Laser Dentistry - Is it Better Than Traditional Dentistry?

I enjoyed my trip to the dentist today. No, really, I did. I spent twenty minutes in the chair, getting my teeth cleaned with the aid of baking soda, betadine and a wee little laser. I admit that I was initially reluctant to let someone put something that can cut through steel into my mouth, but Dr. Kassa really knows his stuff. And this is from the viewpoint of someone who used to prefer extractions to fillings, because at least I got knocked out when I had teeth pulled.

When I was a child, I went to a dentist who actually had leather straps on the arms of his chair to restrain his young patients. By the time I was ten, I was so traumatized from having three fillings and two extractions without painkillers or novacaine that I started shaking whenever I saw a toothpaste commercial.

Except for having an impacted wisdom tooth extracted when I was in my twenties, I avoided dentists until our kids arrived, when I realized that I had to Set A Good Example. Sheesh! If I'd realized this, I might have remained childless. But because we did have kids and the only other option was being a horrible warning and that didn't appeal to my maternal instincts, I bit the bullet and went to the dentist that we took the kids to.

He was very reassuring, kept asking me if I felt any pain and did I need to take a break. He hadn't even put his gloves on at the time. I guess my apprehension showed. He soldiered on though, and managed to do an exam, which told me what I already knew - my teeth were in pretty bad shape. Then he turned me over to his hygienist, who spent an hour scraping my teeth.

Finally, when it was done, I breathed a sigh of relief until she told me that she'd only done a quadrant. There were still three quadrants to go.Not only that, but by the time she was done with the last quadrant, it'd be time to do the first one all over again. Then she added insult to injury by telling me that even this might not save my gums, which were not at all well and would probably need "deep scaling and planing". I didn't like the sounds of that! This was my reward for finally facing down my fear and submitting to these barbaric procedures?

I went twice more, but each time, my blood pressure went higher and higher and all I could think of while I was being worked on was the fact that my maternal grandmother had died from a stroke she suffered in the dentist's chair. I'm a strong believer in genetics as destiny, and I didn't want to go the way of Granny, so I started doing what I always do when faced with a dilemma. I started looking for alternatives on the Net. That's how I found Dr. Kassa.

Dr. Kassa has embraced the future. He has at least three lasers in his office and if there's a drill, I have yet to see it. He says my gums aren't as bad as I thought they were and gave me the equipment and knowledge to keep them from getting any worse. In addition to a really good electronic toothbrush, I use an irrigator with baking soda in the water to zap the little anaerobic bacteria that are trying to colonize my gums. Dr. Kassa is sound on anaerobes. He knows how to get at them with his lasers, which scraping doesn't do, and he knows how to show his patients how to get them with their toothbrush, baking soda and irrigator.

It amazes me that - in this day and age - people are still using these archaic and inneffective methods to practice dentistry. Would we put up with our doctor removing a splinter by digging it out with a knife? No, we expect that they'll use tweezers and a topical anesthetic if it's needed. If we break our arm, we're probably going to have something to say if the doctor goes outside, chops down a cherry tree and ties our arm to a straight piece of it.

Of course, laser dentistry is more expensive than the status quo, which is none too cheap, itself. However, if more dentists practiced laser dentistry and used baking soda and betadine and/or an ultrasonic cleaner instead of the usual manual method of gum cleaning, the price would come down pretty fast because of competition and a wider market for the training and equipment needed to practice this kind of craft.

The biggest upside to this would be that a large segment of the population - the "tied down for dental work as a child" contingent, for instance - would go to a dentist and head off serious oral problems, which could lengthen their lives. Oral inflammation can lead to heart problems and a host of other ailments, so nipping it in the bud is a really good idea.
